Track changes and export assessments
See every change made to an assessment, restore earlier versions and export assessments for audits and reporting.
Regulators, auditors and customers increasingly expect you to show not just your assessments, but how they were reached and how they changed over time. MineOS records every change to an assessment, keeps a version history you can restore from, and lets you export assessments for audits and reporting.
View the activity log
The activity log records who changed what, and when.
- Open the assessment.
- Click ⋯ (More actions) in the top-right corner.
- Select View activity log.

The log opens in a side panel, grouped by day with the most recent changes first. Each entry shows the date, who made the change and what changed. Changes can come from team members, collaborators, Mira AI agents, API keys and automated workflows.
The log covers three kinds of changes:
- Assessment actions, such as creating, exporting or cloning the assessment, changing its status, owner, due date or labels, inviting collaborators, and comments.
- Content changes, such as data sources, data types, server locations or risks being added or removed. Hover over a change to see which question it belongs to.
- Structure changes, such as sections, questions or descriptions being added, removed or reordered.
Filter the log
To focus on specific contributors, click the filter icon, select one or more people or sources, and click Apply.
To see the history of a single question, open the ⋯ menu on that question.

Use version history
Every time an assessment is saved, MineOS creates a new version. [approve: limitation: version history is kept for up to one year]
Preview a version
- Open the assessment and go to Versions. [confirm location]
- Select a version to open a read-only preview of the assessment as it was at that point.

Restore a version
- While previewing a version, click Restore.
- Confirm that you want to restore it.
Restoring replaces the assessment's current content with the selected version and can't be undone, so check the preview carefully first.

The restore itself is recorded in the activity log, so your full history stays intact.
Export assessments
There are two ways to export, depending on whether you need a list of assessments or the full content of one.
Export your assessments list
Export the assessments table to Excel or PDF, for example to share an overview of your assessment program with auditors or leadership.
- In the left navigation, under Governance, select Assessments.
- Filter the table to the assessments you need, or apply a saved view. The export only includes the assessments currently shown.
- Click [confirm button label, e.g. Export] and choose Excel or PDF.

To export the same set regularly, for example all vendor assessments or everything completed this quarter, save the filter as a custom view first. See Organize assessments with custom views.
Export a single assessment
Export one assessment, with its questions and answers, as a PDF to share with auditors, regulators or customers.
- Open the assessment.
- Click [confirm: ⋯ (More actions) or an export button] and select [confirm label, e.g. Export to PDF].

Exports don't include the evidence behind Mira AI suggestions. [approve: limitation]
Who can see the history
The activity log and version history are available to team members within the Portal. Collaborators can't view the activity log, or view or restore versions, nor export.
